In order to migrate your account from one device to another, follow the instructions on the form located here. You'll need your account username, password, and device identifier in order to make the switch.

Need to find your device identifier? Here's how:

Connect the device to your Mac or PC.
On the "Summary" tab in iTunes, click the "Serial Number" label. You'll notice "Serial Number" changes to "Identifier".
Click on "Edit" in the menu at the top and select "Copy".
Now you can paste the device ID into a reply.
